- Subject Line: NEW REPORT OF LOSS, Named Insured, Policy Number, DOL, Claimant (if Applicable)
What to Include
- Acord Loss Notice: Make Sure to Include Insured Contact Information
- Supporting Loss Documentation (Ex: Pictures of Damage, Police Reports)
Reporting Requirements
- Notice of Claim should be made as soon as possible
- Please reference your policy for complete details relating to reporting requirements in connection with claim(s) or potential claim(s), including what to include in your notices.
